The
following are provided by funding through Lake County's Family and
Children First Council

A comprehensive
child development program provided to families with children under
the age of 3. Services include:
Newborn Home
visits: Provides a home visit by a registered nurse to mothers
after the birth of their child. Targets first time and teen moms.
Service Coordination:
Provides a developmental evaluation to families who have a concern
about their infant or toddler's growth or development. Connects families
and pregnant women, whose children have diagnosed delays or are at
risk for delays, to available resources in the community.
Home Service
Provision: Provides
home visits to families receiving Service Coordination to assist those
families in raising healthy children. Home visitors help families
with parenting issues, child behavior problems and a range of other
educational, health, social or mental health issues.

CHILDREN'S COMMITTEE
Provides service
coordination, upon referral, for multi need families, whose service
needs are such, that they cannot be provided through one system.
Develops and monitors
Community Service Plans based on individual family strengths and needs,
utilizing creative solutions based on intersystem cooperation
DISPUTE RESOLUTION
Provides a mechanism
for families and service providers to resolve disputes related to
child/family assessment, the Community Service Plan, or the assignment
of responsibilities to implement the service plan.
OTHER SERVICES
Provides programs,
marketing and outreach to the community, as funds become available,
to promote wellness for families and children and prevent child abuse
and neglect.
